Annette Lujan: Woman Dies in CSU Long Beach Elevator Accident [UPDATED]

Annette Lujan reportedly attempted to exit a CSULB elevator just as it began to move.
The victim has been identified as 48-year-old Annette Lujan, a resident of Huntington Beach. She had worked at CSULB's grants and contracts office for nine years. More details at the bottom.

A woman was killed this morning in an elevator accident at the CSU Long Beach campus. "I can confirm that there was a death, and it was an accident," an officer at the campus police department tells the Weekly.

Local station ABC7 first Tweeted the news of her death at about 10:20 a.m. Also on Twitter, one L.A. resident wrote that the accident occurred in the Foundation Building.

The CSULB media-relations office says it can't confirm anything yet.

"Authorities responded to the campus at 9:10 a.m," ABC7 reports. "The woman was declared dead at the scene." Here's the floor plan for the Foundation Building, which is filled mostly with administrative offices.

Update, 10:45 a.m.: The following email alert was reportedly sent out by CSULB officials just moments ago.

"There has been a fatality on the CSU Long Beach campus today as the result of an accident. The matter is currently under investigation. The name of the deceased is not being released at this time pending notification of the next of kin."

One student named Wyatt Tweets that the victim was "apparently a teacher," but no official confirmation there.

Update, 11 a.m.: University spokeswoman Toni Beron says that "this morning at 9 a.m., we received a call to university dispatch that a woman was trapped in an elevator." She says police immediately arrived on scene -- along with the Fire Department -- and began "administering first aid."

What, exactly, was wrong with the elevator? Beron says that's currently "under investigation." The Long Beach Press-Telegram has more details:

A female employee of California State University Long Beach was found dead this morning at about 9 a.m. inside an elevator shaft at the campus foundation office, authorities said. ...

The woman was discovered on the third floor of the building, on the south side of campus.

Update, 11:15 a.m.: A Long Beach Fire spokesman tells ABC7 that the victim "apparently fell down the elevator shaft" of the administrative building.

An anonymous employee at CSU Long Beach says in an email that "the victim was not faculty, but a respected staff member." Below is a letter from Mary Stephens, the school's vice president of administration and finance.

Dear Campus Community:

I am very sad to inform you that this morning, due to an unfortunate incident at the Foundation Building, a member of our campus community has died. At this time, we are notifying family members and a complete investigation involving University Police and the Los Angeles County Coroner's Office has been initiated.

The university has endured a terrible tragedy today. I know all of us extend our deepest condolences to the victim's family, friends and co-workers.

A counselor with the Employee Assistance Program is available for employees. They can contact Corrie O'Toole at 562/985-7434.

The Daily 49er reports that "employees and students were evacuated after they heard a scream coming from the third floor elevator, according to a Foundation building employee."

Update, 2:10 p.m.: Long Beach fire Captain Rich Brandt tells City News that investigators believe the elevator got stuck between floors, "and the woman tried to climb out but was crushed when the elevator shifted."

Update, 3:30 p.m.: The woman was reportedly dead before firefighters even arrived. When they did, Brandt tells the Press-Telegram that they "worked for more than an hour using a rope and pulley system to lift the elevator and free the body."

The local newspaper described a horrific scene outside the accident site, where the victim's fellow employees hugged each other and cried. Meanwhile, CSULB's media-relations office is denying there was any way to predict the elevator's fatal malfunction today:

The three-story building is about 20 years old and has two elevators, Beron said. She said the elevator where the accident occurred had no reported problems before [the] incident.

Update, 7:30 p.m.: The university has released the following statement, identifying the victim as Annette Lujan.

This morning a Cal State Long Beach employee was fatally injured in an apparent elevator malfunction in the CSULB Foundation building. Annette Lujan, who was the sole occupant of the elevator, attempted to exit the elevator after an apparent malfunction that occurred between floors. Initial information indicates that the elevator started to move as she was attempting to exit. University Police and the Long Beach Fire Department responded immediately to a call of a woman trapped in the elevator, but Lujan succumbed to her injuries at the scene. The details of the accident are under investigation. Lujan, 48, a nine-year employee of the university's grants and contracts office, was a resident of Huntington Beach.

we see that accident happened for women on beach. unfortunately accident happened any where . like so many people are in the office and door fall down one  person come there and door fall down on head .I thing that case urgently meet a doctor and also meet lawyer.The lawyer give a suggest you.

In my senior apartment building I notice that people will put an object at entrance to elevator to keep it open so as deliver items and hurry back and go to next floor and continue on up. i have even put an object to prevent elevator closing temporarily BUT NEVER AGAIN will I ever tamper with an elevator.There should be rules in the elevator telling people what they should not do. People WILL read it I am sure. This is very serious. I remember in the 1950's that there were elevator operators in each elevator. I guess those jobs are gone now. We are living in a fast generation.

Please don't make any assumptions about the state of mind of the victim. No one really knows what actually happened. The victim attempting to exit on her own is speculation only. She may have been in the process of stepping out, when the elevator floor dropped down, that she may have fallen forward, half of her body on the third floor, and half caught in the elevator...unfortunately, it just takes less than a second to be pinned down in between the floor and the elevator roof. I could just imagine the instantaneous event that nobody expected...this is a reminder that when the time comes to exit this life, no one can control it, except God.

I am thinking the same thing you are. Everyone is just making assumptions, no one really knows what happened. What if she was just exiting the elevator, like anybody normally would, and the elevator just dropped on her. Nobody knows except for the victim. So I suggest, for all those who have negative comments, to just keep their comments to themselves. My condolences to Annette's family and friends. May she rest in peace

My daughter goes to school there at CSULB and she called me the day of the accident. This was before any of the details were known. The fire trucks had passed her on the way onto campus. People like MMMMmmm who post erroneously and hatefully are simply narrow minded and callous. He/she doesn't know the circumstances of why the woman decided to crawl out. According to a woman my daughter spoke to who had used the elevator right before the accident, it had been making jerking motions (sort of bouncing).  Maybe the deceased feared injury due to this malfunction? Perhaps she thought it would fall with her in it? Maybe as the thought crossed her mind - the doors opened and on instinct she made a run for it so to speak?We simply don't know how this transpired. Do we even know that she didn't use the "alert" button or the phone? Regardless of whether she made the right decision - a woman is dead. A woman who was loved and cherished by her family and friends, and this callous A-hole would have been less of a waste of space if he'd have just kept his ugly mouth shut.Having nothing good to say about another human being who's crime was dying in a freak accident, (regardless of the circumstances) is very disheartening. It's mind-boggling to say the least. My heart goes out to the people that loved her! RIP sweetheart.

Really a very sad story.  My heart goes out to you on the loss of your sister.  I lost a sister too and it is a very painful experience.  I went to CSULB and graduated in 2001 with my MBA so when I saw this story I was really terribly sorry.  God Bless you.

If it was a 3-story building, this was probably a hydroelectric elevator, These are the kind that use huge hydraulic rams, like the hydraulics that move the arms on a skiploader. 

Those don't have brakes, as the hydraulics basically make it impossible to crash, like a cable-lifted traction elevator would. It is practically impossible for that kind of elevator to do anything but slowly sink to the ground if there's a mechanical failure.

This is a terrible accident which took a human life. It's tragic. However, even with braking systems and emergency systems, accidents and mechanical or electrical failures can still happen. It's a state law that all elevators are to be inspected at least once a year by qualified elevator personal. An inspection typically checks the overall operation, checking various elements of the elevator. Any elevator mechanic (who knows elevators best) will always say that if you're stuck in an elevator, stay inside. For what ever reason why she left the elevator (probably thru the escape hatch, because there are door interlocks that prevent the car doors from opening between floors. The car brakes are only applied when the car stops at a floor or when the "safety system" is engaged), this was a bad set of circumstances to leave without professional assistance. An elevator is still an electro-mechanical machine that can fail at any time. Even after an inspection. Very sad. She was in the wrong place, at the wrong time.

As an elevator mechanic for the last 15 years, i feel the need to give my two cents. The escape hatch is bolted/locked shut to prevent people from climbing out and these tragic instances from happening. If this was an older elevator built/inspected, and maintained according to older safety codes, she very well may have been able to pull the interior doors open. NEVER A GOOD PLAN! It appears that in her haste to exit the car unassisted, a major malfunction happened and crushed her. PLEASE WAIT FOR ASSISTANCE FROM TRAINED PROFESSIONALS!

Very tragic news.

I am a CSULB student who works on campus and have visited the Foundations building plenty of times. I've also gotten on that elevator numerous times as well. Wow...to think the poor woman did not anticipate her impending demise that morning...it's really sad and gut wrenching. One minute you're here, the next you're gone. Life really is too short, so everyone please make the best of it. 

I am so sorry for her and her family. My condolences to her loved ones.

so sad. My heart goes out to to her family, and friends. Mostly to her family, I was a CSULB student up until last spring. So many times I nearly got run over by cars running stop signs there, plus the other scary things that have happened there while I was a student. The campus really should do something about safety, They raised tuition fees last year too. They should use some of the money they get from raised tuition prices on improving safety which should be a top priority for any College/University, or any school. .

I am so freaked out by today seriously 100 percent! Especially because when I came out of class at  9:15 a.m. I saw the ambulances and police at wrapping Do not cross tape. THe woman next to me told a woman was found dead in the third level elevator. I felt so much sorrow for the situation. To make things worse I was crossing the street at CSULB  by the dorms and a bus stopped to let me pass and as I was about to pass the lane next to the bus a car ran the stop sign and was one step from running me over. I need had such a close brush with death. I am so thankful for my life today seriously, but 1000 percent concerned with safety at CSULB.

man that makes me mad they raise the tuition for college and they cant even put some of that money towards sturdy buildings and equipment such as an elevator that probably was used everyday our government and the people in charge of the schools budget need to sit down and spend some of that money securing college campuses to make them safe for all

Yeah, you know I think the public needs to demand transparency on this one; what was there PM (Preventative Maintenance) program---WAS there a regular PM by facilities management? WAS it being kept up?  What was shown during the last PM?  What did their COMPLAINT file show?  Do you they even have complaint files?  If so, are they reviewed--if so, what was found on review of the files of late?  I'll bet money they had received complaints, and so on, and so forth.  This isn't about pointing fingers, it really isn't.  It should be about a thorough investigation---not just of what happened, but WHY it was not prevented via regular inspection and pre-accident repairs as needed.  It is always amazing, following a major accident, that it is usually found it never should have happened.  I'm also sure the family lawyer will be doing plenty of research before making his/her move, and the payment will ultimately come from the people.  For her family, I won't mind if they end up owning the building before this is over.

@Jmichaelisbell, you cant fix stupid. This woman had no common sense. She tried to crawl up to the 3rd floor when the doors opened. The elevator was between floors. She was squashed from the waist down when the elevator started going down as she was almost out of it, is what I have read.I would imagine that part of her was on the 3rd floor and the rest on the walls leading to the lobby or first floor when the paramedics and fire crew arrived. There are phones and emergency call buttons in elevators. She should have used them, or used her head anyways instead of putting herself in harms way. It was her fault and nobody else. Tragic loss of life.

That was an  OLD OLD ElevATOR and isn't like the newer ones lady have you gone there and checkd it? There was no red button or phone in there. Yah she panicked but furthermore that elevator wasn't even contracted by the School university so they even had to call the company that dos the maintenance on it. The school couldn't even open it and one guy there on the oppsite side of the elevator was trying to pull her out when she was half way out thats what killed her as soon as the elevator moved and decapitated her head. Thats why it took over an hour to get her body out after waiting for the company that runs that elevator. I hope the family sues the hell out of school the other elevator was being workd on at the same time also. bad shape for all!

That is what that BIG RED BUTTON is for in the elevator.Most of those have a word EMERGENCY or HELP in BIG WHITE LETTERS on top of the button, just to make it easy to read. She worked there for 9 years. She must have seen it at least once. Anyhow. No sense beating a dead horse. She won't be making that mistake again. The gene pool grows stronger everyday. Nuff said.

Even the best equipment falters from time to time; this accident doesn't play into the tuition argument.  Unfortunately, this is a sad lesson in safety: not to attempt self-rescue from a stalled elevator.
